Pregnancy Outcomes in Sickle Cell Disease (SCD)

Abstract

To assess the influence of homozygous sickle cell disease (HbSS) on pregnancy outcome, an historical cohort investigation was conducted among 9,310 African-American women who delivered at our medical center (USAMC) during the years 1991-1996. An ob/gyn electronic database for this period was cross-linked with USAMC hospital information systems records to identify maternal hemoglobin status by ICD9 codes. The MEDICAID, USAMC medical record, USA Sickle Cell Center electronic databases were also surveyed for confirmation of patient information. For repeat pregnancies, the latest pregnancy was selected for analysis. The distribution of the maternal sample was as follows: homozygous HbSS (N=50), heterozygous trait (HbAS) (N=336), other hemoglobinopathies (N=29), and control population (HbAA) (N=8,895). Neonates born to HbSS mothers had lower mean birthweights (p=.004) than HbAA mothers. HbSS mothers were also more likely to deliver at earlier gestational ages (p=.022). Perinatal + prenatal deaths were more prevalent among HbSS mothers (p < .001), as were rates of pyelonephritis (p=.011) and pregnancy-induced hypertension (PIH) (p=.027). Multivariate analysis identified a constellation of 3 variables distinguishing HbSS from HbAA mothers (P < .00002): low birthweight, pyelonephritis, and PIH. With a few notable exceptions,our observations confirm reports from previous studies of sickle cell disease and pregnancy outcome.
