To evaluate the risk of future infarction in patients with postural cerebral hypoperfusion (PCH), we examined 108 patients with or without hypertension (HT) and with or without diabetes (DM) using upright Tc-99m HMPAO brain SPECT. The patients were divided as follows: Group A, HT(-)DM(-); n =32, 61.8+/-13.4yrs; Group B, HT(+)DM(-), n = 39, 63.3 }10.3yrs; Group C, HT(-)DM(+), n = 13, 62.8+/-9.45yrs and Group D, HT(+)DM(+), n = 24, 66.9+/-8.7yrs. Angiographical carotid stenosis ( > /=75% = 1, < 75% = 0) and the size in the low density areas (LDA) in X ray CT ( > 1 cm in diamter =1, 1 to 0.3 cm =0.5, < 0.3cm=0) were scored in each group. Positive(+) or negative(-) PCH in Group A was determined with visual inspection by the consensus of three doctors. Angiographical carotid stenosis score was 0.03+/-0.176, 0.231+/-0.427*, 0.308+/-0.480**, 0.25+/-0.442*, the size in LDA score was 0.063 }0.168, 0.231+/-0.411*, 0.26+/- 0.439*, 0.313+/- 0.412**; in Groups A, B, C and D, respectively (*p < 0.05, **p < 0.01 vs Group A). The occurrence of cerebral infarction significantly differed between PCH(-) in Group A (n=13) and Groups B, C and D (p < 0.05) but not among PCH(+) in Group A (n=19) and Groups B, C, D. These findings indicated that the risk of cerebral infarction in patients with PCH(+) but without either hypertension or diabetes is equivalent to that in hypertensive or diabetic patients.
