Anne Rochon Ford, Community Co-Director, National Network on Environments and Women's Health, York University. Anne is a writer and consultant on women's health issues. She was co-founder of the Toronto Women's Health Network and DES Action Toronto and has been involved in efforts to establish the Canadian Women's Health Network. She has served on the Interim Regulatory Council on Midwifery and the Transitional Council of the College of Midwives (Ontario). Her current areas of interest include breast cancer, health issues for women with disabilities and health issues for francophone women in Ontario.

To register, please complete the attached conference registration form and return it with your registration fee. Early registration deadline is June 3, 1997. Registration fees include all nutrition breaks, two lunches, a networking breakfast and conference materials (including synthesis papers). All taxes are included.
Refunds:
Registration fees are refundable less a $25 administrative charge if written notification is received by June 13, 1997. No refunds after June 13, 1997.
GETTING THERE:
Travel by air: Air Canada is the official airline for the conference. Call Air Canada at 1-800-361-7585 and take advantage of special rates for conference travel. Please ask your ticket agent to ensure that the Fifth National Health Promotion Research Conference event number CV971355 appears on the tour code box of your ticket regardless of the fare purchased. The Halifax International Airport serves most major airlines.
Travel by land:
Via Rail connects from anywhere in Canada and the USA to Halifax. Highways from both the USA and Canada link up to the Trans Canada Highway.
Ground transportation:
Dalhousie University and the Holiday Inn Select are a 30 minute taxi ride from the Halifax International Airport. Taxi fare one way costs approximately $35. The Airbus shuttle service runs between the airport and major Halifax hotels including the conference hotel, Holiday Inn Select - Halifax Centre. A shuttle leaves the airport every 30 minutes and costs $11 one way and $18 return.

CONFERENCE SITE:
The Conference is being held in the Student Union Building at Dalhousie University, 6136 University Avenue, Halifax, Nova Scotia. The building is air conditioned and wheelchair accessible.
Parking:
Parking is available on campus at a cost of approximately $6.45 per day (includes taxes). All vehicles parked in campus parking lots must display a parking pass from Dalhousie University Security Department. Parking passes can be purchased at the conference registration desk upon arrival.

Residence accommodations at Dalhousie University:
Inexpensive accommodations are available in student residences at Dalhousie University. Rooms are limited and therefore should be booked before June 3, 1997. If you wish to reserve a room in a Dalhousie University residence, complete and return the residence accommodation form enclosed.
Hotel Accommodations:
A block of rooms has been reserved at the conference host hotel, Holiday Inn Select - Halifax Centre from Thursday, July 3 through Saturday, July 5 at the special conference rate of $97/night plus applicable taxes. Rooms should be booked before June 3, 1997 to take advantage of the conference rate. FRIDAY EVENING LOBSTER FEAST (optional):
Join us on the historic Halifax waterfront for a traditional Maritime lobster feast. The feast will be held on Friday, July 4 at 7 PM at Murphy's on the Water. Dinner includes a 1.5 pound lobster in the shell, soup or tossed salad, creamy potato salad, cole slaw, rolls, dessert, and tea or coffee. Alternatives to lobster include barbecued steak or vegetarian pasta. Cost is $32.95 + $4.29 gratuity = $37.24.
SATURDAY EVENING HARBOUR CRUISE (optional):
The perfect way to end a Saturday....Enjoy a spectacular view of the city's waterfront and McNabb's Island on a 2 hour evening harbour cruise.
