Employee Acknowledgement Requirement

In order to follow accepted best practices of IT security awareness we are asked by McMaster's external auditor to regularly remind University computer account holders that they are expected to use University computing and networking facilities in accordance with the Policies governing use of UTS facilities, in particular the McMaster Code of Conduct for Computer and Network Users, as well as applicable Senate policies.

The use of McMaster's administrative computing systems (MVS and Oracle) is governed by the Guidelines on Access to Information and Protection of Privacy policy.

If you have an account on one of McMaster's administrative systems, you are required to complete the Employee Acknowledgement on Confidentiality and Computing Access as a formal indication that you have read and understood these policies.

Please print the form Employee Acknowledgement on Confidentiality and Computing Access, complete it with your name, employee id number, date and signature and return the completed form via campus mail to Data Security T13-101B. If you are unsure whether you have already completed one of these forms or not, please complete one anyway and send it in.

If you are unable to print your own copy of the document, you may request a printed copy of the form by contacting Data Security:
