Medical Monitoring Program

McMaster has changed its medical monitoring program for BSL 2 biological agents effective July 1, 2011. The supervisor has the responsibility to ensure implementation.

Listed below you will find medical information sheets for the BSL 2 agents which the Presidential Biosafety Advisory Committee has recommended for medical monitoring.

In addition, the Canadian Immunization Guide should be followed.

If any individuals under your supervision are using any of the agents:

1) Print a copy of the information sheet for each individual and have them read it.

2) Both the individual and the supervisor initial and sign the last page.

3) Keep a signed copy in your biosafety files.

4) If your faculty/staff chose to become part of the active medical monitoring program, or have any questions, they can contact the Occupational Health Nurse at gibbong@mcmaster.ca or call the office at extension 20310 or fax 905-528-0449.

5) If your graduate students chose to become part of the active medical monitoring program, leave your name and contact information with the Biosafety office in room 1J11 or call extension 24956 or cassidk@mcmaster.ca. As soon as the grad student medical provider is formalized, you will be contacted.

6) This documentation will become part of the annual biosafety audit.
