PHAC/CFIA Forms To Accompany Human/Animal Pathogen BSL1 Imports
If an agent is a human or animal pathogen and rated as BSL1, you do not require a permit for importation. However, if you wish to have forms in place to assert this at the border, you have two options:
(1) Complete PHAC import permit for your BSL1 agent and receive back a Letter of Exemption.
(2) Complete Status form below and fax it and additional information to PHAC to receive a letter from PHAC stating the item is BSL1 and does not require an import permit.
Again this may not apply to plant pathogens, aquatic pathogens or foreign animal disease agents.

From PHAC:
"In order to facilitate border clearance, our office can issue you a Notice Letter indicating that a permit is not required. If the importer wishes to obtain this letter, please provide the following information:
1. Name, address, phone and fax number of the importer.
2. Description of material being imported.
3. Name and address of the supplier.
4. Signed Statement of Status form ... which must be completed by the importer."
