Documentation Requirements

Documention requirements for working with bioazards can be quite complicated. In order to streamline and simpify the process we suggest the following organization/filing strategy: The SOP/Training binder, the Biological Inventory binder and the MSDS binder. Items that can be filed are ones that do not need to be accessed often, but kept for a certain period of time. These would be PHAC/CFIA checklists, import permits, BSC certification, equipment decontamination or calibration records etc.

The SOP / Training Binder

Orientation Section - This would consist of a detailed list of items the supervisor would go over with a new worker in the laboratory. Typically introduction to lab and department staff, touring the facilities, review of responsibilities, list the training required and most importantly 'reading requirements'.

Training Records Section - This would be one training record per worker. It is convenient to have all the signatures near the beginning of the binder rather than peppered thoughout. It should list:

  • worker information
  • required training from outside the laboratory, each dated and signed by worker and supervisor
  • required training from inside the laboratory, each dated and signed by worker and trainer/supervisor
  • the 'reading list', each dated and signed by worker and supervisor once complete
  • Documentation of Lab Specific Biosafety Training <--- very nice template

 

SOPs / Documents Section - This section contains the 'reading list' or all the documents that the new worker is supposed to read and understand. After each document is read, the supervisor ensures that the worker understands the document, and both sign off on the training record. The reading list would be determined by the supervisor.

  • SOPs required by PHAC/CFIA
  • other lab-specific SOPs
  • guidance documents
  • equipment safety
  • lab rules or job list
  • .....

The Biological Inventory Binder

Instructions section- this would allow someone who does not work in your lab to understand how your inventory is organized and if asked, that person would be able to locate an item. Or if a vial was taken from a spot, the person would be able to use your inventory system to identify it.

Inventory section - This section is the inventory in the format you wish. Templates and examples can be found from the menu on the left "SOPs / Docs / Forms & Templates". If you would like to add your template to that list, email us.

The MSDS Binder

Table of Contents - This section allows a user to determine if the chemical is there or not. It also allows you to list the chemicals each lab has if they are shared.

Alphabetical List of Chemicals - This will be the actual MSDS sheets obtained from the supplier or the internet.
